Presentation Mobile Emergency Service for Resuscitation and Extrication

The interventions that SMURDis responding to include all emergencies that place human life in immediate danger. These are car crashes, explosions, labor or household accidents like falls or electric shock, states of unconsciousness which include cardio-respiratory arrests and not the least, the accidents involving many victims. The unit intervenes regardless of the victim's age, approximatively 7% of our cases being children under 16 years old.

Besides the common interventions which use mobile intensive care units, the SMURD staff has used many times special airborne intervention units. These were different tipes of helicopters and airplanes used to transfer critical patients from Tirgu Mures to other health care centres in Romania or abroad, or from other hospitals to Tirgu Mures. In these cases the medical crew consists of two staff members, one of them being a physician specialized in emergency and intensive care.

Once the patient's condition is stable in pre-hospital, the next step is to move the patient to the Emergency Department where the treatment is continued. In case that the patienit's condition is critical, he/she will be admitted to the Intensive Care Room, which has two beds permanently prepared no matter the patient's age or condition. Less critical patients are admitted in the room destined to immediate treatment or to the short term observation room. Both these rooms have 5 beds, together with the EKG monitoring system. In the Emergency Department there are permanently one shift-leader physician together with other two physicians, helped by the shift-leader medical assistent and a number of 4 to 6 volunteers including Medicine students, Sanitary School students, etc.
The Emergency Department is designed to continue the treatment begun in the street, at the accident site, or to begin the treatment if the patient has arrived with a non-medical vehicle, to diagnose the patient in cooperation with the specialists in the hospital and to observe the patient until the hospital addmittance or until the release, in case the patient's condition does not require hospital treatment.

The Emergency Department admits patients from three sources: SMURD, the County Ambulance Service and private means.
Fortunately, in Tirgu Mures the percentage of critical cases transported with private vehicles is very low compared to the rest of the country, being below 10%. Approximatively 50% of the critical cases arrive with SMURD's mobile intensive care units while the rest arrive with one of the ambulances belonging to the County Ambulance Service.

The staff working with SMURD and the Emergency Department includes:

8 physicians employed for this service by the Mures County Hospital;
*
4 emergency medicine residents;
*
2 residents specialized in anaesthesiology and intensive care;
*
10 psysicians employed by the Sanitary Direction;
*
8 medical assistents employed by the County Hospital;
*
6 firemen / drivers employed by the Internal Affairs Ministry for the mobile intensive care unit and for the mobile extrication unit;
*
1 medical assistant in chief employed by the Internal Affairs Ministry;
*
1 medical assistent on military duty at the Mures Fire Department;
*
Approximatively 60 volunteers, students, medical assistents, students at the Sanitary School, etc.;
All the personnel mentioned above, including the firemen, is specially trained in emergency medical assistance, by training programs and follow-ups, according to each one's level of qualification.
