The incoming call is answered by the joined dispatch situated at the City Hall of Tirgu Mures, on the 112 emergency number or on the 961 phone number. Sometimes the calls can be forwarded by the Fire Department dispatch, answering the 981 number. These calls usually announce fires, explosions and accidents.
The integrated medical dispatch has a permanent radio connection with the Fire Dept. dispatch. The latter has a permanent radio and phone connection with the Police dispatch and with the Civil Protection dispatch.

number, the staff at the dispatch will evaluate the case and will decide what type of medical unit is best to intervene. The majority of cases are directed to the County Ambulance Service, including transports, house calls and emergencies. If the case signalled to the dispatch involves one or more victims in vital danger, due to either the sudden occurance of a medical condition, like a cardio-respiratory arrest, a myocardial infarct, a coma, etc., or to severe trauma, regardless of it's cause, such as: accidents, falls, etc., the dispatch will alert the SMURD unit on call. |

In this manner the critical patient is provided with an emergency medical assistance in pre-hospital at a very high level, using a large enough crew which is also trained for this kind of cases, under the leadership of an anaesthesiology - intensive care specialist or a physician specialized in emergency medicine.

The unit can be alerted by the Fire Department dispatch or directly, by radio. During day-time, the unit has two components, one at the hospital in a very fast intervention vehicle, with a doctor and his assistant, and the other at the Fire Department, with a fireman / driver and two trained persons, who can be medical assistants, students of the Medical School, Medicine students or firemen with training in emergency medicine.
During night-time, the physician is based at the Fire Department, being a part of the emergency unit consisting also of a fireman / driver and dtwo trained persons.
In the first case, both vehicles are alerted and they meet at the scene; this system is called "rendez-vous". This system makes possible, in case other emergencies are called in, for the doctor to go to the next case to support the other medical unit, while the first emergency unit transports the victim to the hospital. Another good thing about this system is that, during day-time, when the number of cases is larger, the doctor stays at the Emergency Department and works there when he's not called to emergencies outside the hospital.
In case of major accidents, with multiple victims, the dispatch will allert the entire SMURD, which will gather together a number of at least 24 people, including about 9 physicians. A similar system allows the paging of a number of people at the County Ambulance Service, and the Fire Department alsa can page and gather another team of 20-30 people.

In case of specific incidents, like car crashes with immobilized victims, the mobile extrication unit is also alerted, in order to offer the necessary technical support. In case of drownings or drowning imminence, the rescue boat at the Fire Dept. is also ready to go.
Once the crew is in position, it is lead by the physician in charge or by the SMURD medical coordinator, or by the next-in-charge.
Emergency medical assistance is considered to be a team-work and every member of the team, including the fireman, have an important and specific part. In case technical support is required from the extrication unit at the Fire Dept., the cooperation between the medical unit and the extrication unit has to be perfectly harmonious. This is the reason in favour of creating crews including firemen instead of separate units meeting at the scene for the first time.
